- How is it possible for someone eating a ketogenic diet to go into a state of ketoacidosis? Should I be concerned?
Hi Jimmy and Doc Nally,
I have been a BIG supporter of LCHF and keto for years. I personally follow low-carb and have found it to be the only diet that prevents me from binge eating. It has given me a freedom with food I never knew I could have. I was at brunch with a friend who is a nurse yesterday and was talking about how frustrated I am with the way that diabetes is currently managed by doctors and nurses here in Australia. But many dietitians and medical practitioners are too scared of speaking up about ketogenic diets out of fear of having their license pulled. My nurse friend told me she had a patient who only ate bacon and eggs in very small amounts and had to be admitted to the ER with ketoacidosis with ketones at 7.0 mmol. According to my friend, a dietitian who was involved in her care said to her, "I hate this stupid keto diet, there have been so many cases of this happening in the past few years." I was a bit gobsmacked given that the chance of someone with a normally functioning pancreas developing this seems impossible because insulin would prevent this from happening. I am really perplexed by the whole issue of how ketoacidosis could possibly happen in someone eating a ketogenic diet. This just doesn't make any sense to me. I'd love it if you could discuss this on your show.

Hey, what's up guys? I’m a 36-year old Ketonian from Alaska and have been on the ketogenic diet for the past four months losing a total of 50 pounds. I have a weird question for you that I haven’t seen covered anywhere—can ketosis have an impact on your voice? I’m noticing that my voice has been a bit more raspy since starting the diet where it feels like I’m about to lose it. This always forces me to clear my throat but it has little to no effect. Does this have something to do with my beloved buttered coffee or something else in my ketogenic diet?

2. Is simply cutting carbs and eating more fat in my diet enough without having to go full-fledged keto?
Hey guys, I’m totally new to this whole keto concept and not ready to dive all the way in just yet. I love a lot of what you say on this podcast and I found reading Dr. David Perlmutter’s book Grain Brain was a real eye opener. My question is whether simply reducing carbs a little to more of a moderate carbohydrate diet can make enough difference in my health without having to be so severely limited as happens when you go ketogenic. I’m a bona fide carboholic but not overweight. I just want to be healthier and fitter. If I replace my carb heavy choices with lower carb, higher fat ones instead, can I expect to experience the health gains you talk about on Keto Talk? Maybe I’ll try keto someday if you think I need it.

3. What non-scale victories should I be looking for when weight loss stalls while eating keto? Is a weight set point real or not?
Your podcast accompanies me to and from work during the week and I enjoy all of the great insights Doc Nally has from his clinical experience. In Episode 16, The Doc mentioned that the rate of weight loss is rapid initially (5-15 pounds a month the first 3 months), then slows to a comparative crawl (2-5 pounds a month) thereafter. He said there is a high probability of a weight loss stall and I’m currently in the midst of that for the past year.
I am active on several low-carb message boards and see many, many people get frustrated by this slowing of their weight loss after rapidly losing a lot of weight in the first several months. Dr. Nally also said that if you have a lot of weight to lose, it can take several years to do it. I would love to hear more about this and any recommendations on what non-scale victories I should be looking for to keep me motivated.
I’ve talked about this with my low-carb doctor who says I am at my set point goal weight now and have nothing to worry about. Can you guys address the idea of a set point in your weight and whether I am worrying too much about this? Thanks so much and keep on ketoing on!

I just want to say how much I love your podcasts! You guys just fit together in an awesome way and seem to be having the best time while delivering fantastic information! I am 57 years old and did keto a couple of years ago for about two months and lost 35 pounds. I went to my doctor who wasn’t against keto but he told me this was a difficult diet to stay on. This planted a seed in my mind and I ultimately quit my ketogenic diet because of it. I gained back all of the weight I had lost and have had a horrible time getting going again. My husband wants to eat everything in sight and that doesn’t help me get back on track. He had three stents in November and would greatly benefit from eating keto as well. I feel defeated and just needed to reach out to someone who KNOWS the truth. I am going to start again, but already have the mindset that I will fail. I know that won’t help me, but it’s where I’m at. How can I help myself and convince my 60-year old husband that he needs to do this too? Maybe if I can convince him this will help our love life he’d be willing to give it a try. I have to hit him with something important to him.
