I love your podcast - it's the perfect mix of nerdy science in an easy-to-understand format and I’m excited about the addition of Dr. Will Cole who maybe can help me understand something that’s happening to me right now. I started keto in October last year and I am a Type 2 diabetic who let my diabetes and weight become out of control due to uncontrolled stress the two previous years. With the exception of a few vacations where I've cheated, I've been strictly keto otherwise eating less than 20g of carbs a day. I feel great eating this way, but I’m very slowly losing weight and my blood glucose numbers just aren't normalizing—they’re averaging 200-225 in fasted readings even with taking 1500mg of extended release Metformin. I’m determined to get my diabetes under control without insulin and I’m under the care of a ketogenic-friendly physician who says it takes patience for the blood sugar levels to come down. I'm sure it does, but I'm hoping maybe you guys can explain why it takes so long for blood sugar and weight to come down as a Type 2 diabetic when it seems like I’m not getting any results yet.

1. Would doing a protein sparing modified fast that is low-carb, but also very low in fat and calories bring more effective ketosis than a traditional keto diet?
Hello Jimmy and Dr. Cole,
I recently purchased Keto Clarity and read it with a vengeance. Thank you so much for your insights, experience and expertise included in that book, Jimmy. I now feel like I have a great grasp on nutritional ketosis and how it can benefit the body. I've recently been through a trying period with my health and I ended up losing 50 pounds very rapidly due to not eating and malnutrition followed by gaining back 90 pounds when I was "better" again. I recently visited an endocrinologist to help me lose about half my body weight and I was excited when I left the appointment because she told me that I would be on a ketogenic diet! A friend suggested your book, so I purchased it and read it right away. However, after meeting with the dietitian through her office, I found out that I was put on what’s called a protein sparing modified fast. The plan is very limiting allowing only about 600 calories a day. It is also very low in fat and only includes 11 ounces of protein daily.
So, after much thought and my sad attempt at researching this online, I thought I would contact you to ask this important question: Does limiting the amount of food you eat on a plan like this equal what a bariatric surgery patient would consume put the body into a more effective state of ketosis than following a more traditional ketogenic diet as described in your book?
I truly appreciate any thoughts you may have on this.

2. Is it possible that eating the higher amounts of protein required by the GAPS diet is causing me to plateau and even gain weight?
I recently began working with a nutritional therapy practitioner and have been practicing the GAPS diet for about a week now to begin healing my gut and digestive system as well as to detoxify. Before this diet, I was following a ketogenic approach along with intermittent fasting for 16:8 naturally and then practicing 42-hour fasts three times a week and losing weight fairly easily. Now my weight has gone up some and plateaued. My NTP has asked me to stop fasting since it might deprive my body of the nutrients it needs to heal.
At the first stage of the elimination portion of the GAPS diet, you eat animal meat boiled in bone broth with specifically approved vegetables like squash, broccoli, onion or cauliflower that are also boiled in bone broth. I am becoming convinced my plateau has to do with the higher amount of protein in this approach. I am only able to add animal fat to the stews and soups that are GAPS-approved and realize it's not the larger amount of fat I was eating on keto. So it seems I’m now eating a high-protein, moderate fat, low-carb diet. Do you have any insight whether or not it’s the protein that is stalling my weight loss and even gaining weight again?

Dear Jimmy and Dr. Cole,
Can eating a ketogentic diet mask the symptoms of perimenopause? I have been eating a ketogenic diet for over a year after starting my nutritional health journey with a primal/Paleo diet. The pounds on the scale simply are not moving. My doctor had said according to my health markers that there is nothing wrong with my metabolism. My thyroid and adrenals were good and my A1C is 5.0 showing my blood sugar to be well-controlled. According to my doctor, my sex hormone levels indicate I am still fertile at the age of 46. When I test blood ketones, I am .5 or higher and even seeing as high as 2.2 mmol. Prior to eating clean, my cycles were becoming irregular, but after 2 1/2 years of eating well my cycles are now back on track. I am having cyclical symptoms of fatigue, moodiness, and joint pain that is not in correlation with my cycle. It's seems to be happening every 3-4 months and I am no long losing inches while gaining scale weight. Could these symptoms be perimenopausal even with a regular cycle? I am frustrated I'm not losing weight, but am grateful my health seems to be right on track.

My granddaughter is a healthy 11-year old who is a bone fide carb junkie. She simply cannot resist eating carbage of all kinds. Both of her parents have been keto since March and I wonder if my granddaughter could eat this way as well so long as she gets enough protein. I’m a registered nurse and it seems to be safe for her to do to help get her extra weight under control. My real concern is with her carbohydrate cravings and whether they will turn into an eating disorder. We’ve already caught her hiding candy and cookies so she can eat them later and then she doesn’t eat the real food she is served at meal times. Please help advise me because I want to help my granddaughter ward off future health problems now.
