This year Intermountain launched a life care model for seniors designed to keep them healthy and living fuller, more independent lives while significantly reducing emergency room and hospital visits. In this podcast, Bradley Nielsen, life care manager for Homespire, talks about the benefits of the program to patients and the roles of his team with Mark Briesacher, MD, Intermountain chief physician officer and Jeff McNally, MD, executive medical director for Homecare and Hospice. Homespire is a resource for family medicine and internal medicine providers, geriatricians and other specialists, family members and loved ones to connect seniors with a service that treats the whole person, addresses care gaps and provides non-medical services essential to daily living.
